Prescription information

These services require a prescription: diagnostics, custom equipment, home sleep testing, wound therapy, respiratory therapy, IV therapy, electro-medical devices and supplies, bone growth stimulators, custom orthotics, prosthetics, nursing and home health services, post- acute care, mobility equipment (unless replacement). Prescriptions presented must be dated within one year. A medical report may be used in lieu of a prescription for certain items and services. A ChronovoCare representative will consult with you regarding required documentation and will assist you in obtaining a prescription from your treating physician or specialist. These services do not generally require a prescription: hearing aids and supplies, chiropractic services, massage therapy, acupuncture, physical therapy (varies by state), standard medical equipment, medical supplies, diabetic supplies and equipment, off-the-shelf orthotics, transportation, home and vehicle modifications and dental services.
